Our Coordinated Care Program

Our Coordinated Care Program is designed for individuals with complex mental health concerns.

It involves assessment and treatment with a psychiatrist, psychologist, and social worker/counsellor who coordinate their efforts and work as an interdisciplinary team to provide the highest standard of care and ensure the best possible outcome.

Our program features ongoing and dynamic reassessment, enabling our team to identify opportunities throughout the program to support clients and to have a beneficial impact on their wellbeing and quality of life. The CHS Coordinated Care Program uses a phased approach with personalized support and feedback to achieve client-centred goals.

Our program uniquely offers:

  • Evidence-based therapeutic interventions to ensure optimal results
  • Interdisciplinary approach
  • Personalized treatment program
  • Dynamic reassessment of treatment goals throughout the program
  • Ongoing support and treatment with program extension options when needed
