Our program features ongoing and dynamic reassessment, enabling our team to identify opportunities throughout the program to support clients and to have a beneficial impact on their wellbeing and quality of life. The CHS Coordinated Care Program uses a phased approach with personalized support and feedback to achieve client-centred goals.
Our program uniquely offers:
Comprehensive Assessment & Stabilization
Our team will complete a joint initial assessment, set individualized goals and work to stabilize the client. Our unique approach will incorporate joint psychiatry/psychology clinical sessions, including psychoeducation, coping and symptom stabilization and psychopharmacotherapy.
Custom Therapy
The program’s therapy focus will depend on the client and be tailored to address their specific needs. Clinical sessions will continue throughout, in addition to ongoing psychopharmacotherapy assessment.
Ongoing Reassessment & Treatment Adjustments
Client needs, health risks, and quality of life will be reassessed frequently throughout the program. Treatment adjustments and re-evaluation of goals and support needed are implemented dynamically as they are identified.
Regular Check-ins, Coaching & Support
We support the client throughout the program to monitor, ensure engagement and inform treatments.
Interdisciplinary Team
Psychiatrists, psychologists, and social workers coordinate their efforts and work together.
Checkpoints & Program Completion
Transitioning care and monitoring check-ins will ensure continued client support, including extension options if required. The overall change in recorded evidence-based metrics will determine the achievement of client-focused goals. Client discharge from the program will come with recommendations for ongoing support and treatment.
